This is the current, up to date version of the PA DOT forms: download this version unless you've started using PA DOT forms prior to 6.Jun.2003 (if you were using the PA DOT forms prior to this date, use these forms instead).

PADOTNEW.EXE

To use the form:

• Start by clicking on the above download link.  Your browser will ask where you want to place the download: make sure that you place it into the same hard disk subdirectory that holds your LOGDRAFT program (this is usually C:\ld3 for the DOS version, C:\PROGRAM FILES\GEOSYS or C:\GEOSYS for the Windows version).
  
  
• Once you've downloaded the file, double-click on your "My Computer" icon, or open the Windows Explorer.
  
  
• Using Windows Explorer or the "My Computer" dialog, navigate to the hard disk directory where your LOGDRAFT program is installed: in the right-hand pane you should be able to find the file you just downloaded. Note that the file may or may not end in ".EXE"; for instance, you may see either "PADOTNEW" or "PADOTNEW.EXE".
  
  
• Once you've found the file you downloaded, double-click on it  -- this should open a DOS window and show a number of files being "extracted". If you get any messages to the effect that a certain file already exists, say "y" to the "Overwrite" message.
  
  
• To use the form with LD4: Start LD4 then start a new project.  Choose "Project" > "Default Data Entry Form" and select:
  
  
  • PADOT if you'll be entering DOT boring log forms
  • PADOT12 to enter District 12 DOT boring log forms
  • PAFORM214a to enter the Driller's Boring Log form
  • PAFORMD480 to enter the D-480 Engineer's Test Pit Log
    
  
  
• When you're ready to print, select "Print" > "Reports", select the borings you're looking to print, then click on the "Report Forms" Print Settings tab and select:
  
  
  • PADOT to print the DOT boring log form
  • PADOT12 to print the District 12 DOT boring log form
  • PAFORM214a to print the Driller's Boring Log form
  • PAFORMD480 to print the D-480 Engineer's Test Pit Log
  • PASTRUCT to print the DOT structure tracings form. (This form should be exported to AutoCAD and placed on a standardized plate.) If you'll be printing this form, select PADOT as your data entry form.
  • PASTRUCT(Dist11) to print the District 11 structure tracings form. (This form should be exported to AutoCAD and placed on a standardized plate.) If you'll be printing this form, select PADOT as your data entry form.
  • PAPROF to print a profile report.
    

NOTE: The PADOT and PADOT12 forms must be printed with a vertical scale of 3 feet per inch.
